Treasure Valley Youth Tennis
Participation Waiver and Release of Liability:
By registering my child for participation in any program, clinic, camp, or event offered by Treasure Valley Youth Tennis, I, the parent or legal guardian, hereby acknowledge and agree to the following:
1. Assumption of Risk
I understand that participation in tennis and related physical activities involves certain inherent risks, including, but not limited to: physical injury, illness, falls, contact with other participants, equipment-related injuries, and weather-related hazards. I voluntarily assume all risks on behalf of my child.
2. Release of Liability
I hereby release, waive, discharge, and hold harmless Treasure Valley Youth Tennis, its staff, coaches, volunteers, agents, and affiliated organizations from any and all liability, claims, demands, actions, or causes of action resulting from any injury or illness incurred by my child while participating in or attending any activity sponsored by the program.
3. Medical Authorization
In the event of an emergency, I authorize Treasure Valley Youth Tennis staff or representatives to secure medical treatment for my child. I agree to be responsible for any medical bills incurred as a result of illness or injury during participation.
4. Behavior and Supervision
I understand that my child is expected to follow all rules and guidelines set by the program staff and to behave in a respectful and safe manner at all times. Repeated or serious violations may result in removal from the program without refund.
5. Photo & Video Release
I grant permission for my child to be photographed or recorded during program activities, and for these images or recordings to be used by Treasure Valley Youth Tennis for promotional or educational purposes (website, social media, flyers, etc.), unless I submit written notice opting out.
